what can happen if spindle nut come off while driving?

The Wheel will pop off and take the wheel drum off with it.
Continuing that train of thought, your first response when you see your rear wheel pass you will be to stomp on the brakes. With no drum to hold the shoes on, the pistons in the wheel cylinder will overextend and spray brake fluid all over the place, and there goes half of your brake system. Of course, this won't affect your stopping power a lot because of the "throwing out the anchor" effect of dragging your brake backing plate (and maybe more) on the ground. You can probably fight the wheel enough to get stopped somewhere in the vicinity of the driving lane you were in, but once forward motion ceases, you're done, and it's going to take a hook to get your car home.

But as so many have said, give a good pull on the breaker bar when you tighten it up and you should get away with it.

1 Switch the nuts from side to side.
2 Use a slightly thicker washer to put the deformed part in a different position.

I just used the only punch I have, maybe a 1/4" flat end but it creates a lot off damage. A narrower punch might not be strong enough. Point here is that if you could make a nice neat small depression it would increase chance of being able to reuse safely.

Anybody know a good way?
I use the pointy end of my fenceing pliars.

Yeah, those suckers cost $9 each from napa in 2014. I took a drum off not knowing about this bummer just to investigate a noise. Fortuanately was able to tighten nut more then it had been and so got a new place to dimple. Not too cool though, means someone didn't tighten very well and my other side may be loose.

Still, will need to do brake job soon and that will add 18 bucks to job. Maybe will be able to switch nuts as recomended here but now maybe not.
